The text documents for this page were mostly Microsoft Word
documents in their original form, saved in RTF format and then
converted
to HTML using a shareware program called rtftohtml. The figures were
originally PICT files and were converted to GIF format using
GraphicsConverter1.7.8.
I have not tried any fancy formatting of tables but have just let the
rtf to html converter handle them; it simply puts a <PRE >
in front of a table. Subsequent editings were done directly on the html
file in MS Word 97.

reproduced or redistributed. Some publishers seem quite reluctant
